A check box allows you to give multiple choices to a user. For example, if you wanted to know how many events a volunteer was interested in, you could use several check boxes to allow the user to indicate which events they were interested in.
  1. Click "Check Box" in the left menu of the Form builder



  2. Fill-out the specifics:

    Label - What the user see's on your website, ex) Name
    Name - What you see when editing the form.
    WARNING!- The Name field cannot have any spaces in it or any symbols like -,./. So if you were typing "Sign Up for Amp?" you would have to type "SignUpForAmp" in the "Name" field like the example below.
    Value - Not usually needed.
    Checked - Click this box if you want the checkbox to be checked automatically

  3. Click the blue "Update" button and the form builder will reload with the changes you entered.
